# Scholar Details Form

Collect scholar applications in a simple **Google Form**.\

These are the most common details you’ll need to ask your scholarship applicants before you onboard them:

* Name
* Discord Username
* Facebook Profile Link
* Email
* Location
* Have they applied in other scholarship program?
* What is there EXP Level in Axie Infinity?
* What device/s they’ll be using?
